Metformin, a first-line treatment for type 2 diabetes, acts primarily by reducing hepatic glucose production and improving insulin sensitivity in the liver, muscle, and adipose tissues. This results in lower fasting and postprandial blood glucose levels.
The mechanism by which metformin achieves these effects is multifaceted and still not completely understood, but it’s believed to act in part by inhibiting mitochondrial complex 1 of the respiratory chain<\/strong>, which leads to an increase in the AMP<\/strong>\/ATP ratio. This in turn activates AMP-activated protein kinase (AMPK), an important cellular energy sensor and regulator.<\/strong>
When activated, AMPK stimulates catabolic pathways (those that break down molecules and produce ATP) like glycolysis, beta-oxidation, and autophagy<\/strong>. At the same time, it inhibits anabolic pathways (those that build up molecules and consume ATP) such as gluconeogenesis, lipid synthesis, protein synthesis, and cell growth.
This “energy switch” to a state of low energy can lead to several downstream effects, such as reduced glucose production in the liver (which helps lower blood glucose levels), increased glucose uptake in the muscles, and reduced insulin resistance, among others.
Overall, the effect of metformin is to improve insulin sensitivity and reduce glucose production, helping to regulate blood glucose levels in individuals with type 2 diabetes. It’s worth noting that metformin’s effects are not just limited to its actions on metabolism; recent research also suggests it may have anti-inflammatory and anti-cancer properties.
